Missouri Foundation for Health bestowed a 3 year grant to VNA for its Advanced Illness Management Program. Missouri Foundation for Health is a resource for the region, working with communities and nonprofits to generate and accelerate positive changes in health.
VNA's Advanced Illness Management (AIM) Program focuses on patients with difficulty leaving the home due to serious chronic illness and helps prevent and relieve physical, emotional and social suffering in order to enhance their quality of life.
AIM is the only home-based program of its type offered in the Greater St. Louis area.
VNA is honored to have received this grant to provide care to those who are the most critically ill and to allow us to make an impact in our community.
