Case Manager Hospice RN - Full-Time, Part-Time & PRN
Qualifications
-
Registered nurse in the State of Missouri and a graduate of an accredited school of nursing.

2 years of nursing experience in hospice, home health, and/or oncology. (Similar experience may be considered as it related to the position).

Excellent communication and teamwork skills.

Proficiency in Electronic Medical Records Charting preferred.
Hospice Office/Field Nurse RN or LPN - Full-Time
Qualifications
-
Current Missouri License.

2 years of palliative care or hospice experience.

2 years experience in coordination of services.

Excellent communication and teamwork skills.

Experience using Electronic Medial Records.
Social Worker - Full-Time
Qualifications
-
Applicants must have a Masters’ degree in social work, LCSW preferred.

An ideal candidate will have worked in a medical or counseling setting utilizing excellent communication skills dealing with complex family dynamics and/or have experience in hospice.

Palliative Care Physician (Community Based)
Full-Time or Part-Time
Qualifications

Current Missouri license

Current DEA and BNDD license

Board certified in Geriatrics or Palliative Care preferred or an Internal or Family medicine physician with an interest in geriatrics or palliative care.

Palliative Care Nurse Practitioner
Full-Time or Part-Time
Qualifications

Current APN licensure.

2 years of palliative care, geriatric or family nurse practitioner experience preferred.

Hospice Care Aide - PRN
Qualifications
-
Two years of experience required.

Home healthcare or hospice experience preferred.

Chaplain Hospice & Palliative Care - PRN
Qualifications

Be a respected member of their religious community (ordained, commissioned or credentialed). Prefer Hospital or Hospice experience.

Clinical pastoral education units preferred.

Candidates with a bachelor’s degree with an emphasis in counseling or related subjects will be considered if ordained, commissioned or credentialed with their religious community.

Human Resources Administrative Assistant -Part-Time
Qualifications

Minimum of 3 years of administrative & human resources generalist experience.

Strong administrative skills.

Self-motivated individual with excellent work ethic.

Ability to prioritize and organize work.

Attention to detail.

Proficient in Outlook, Word and Excel; knowledge of PowerPoint and Access is a plus.
