Case Management Consultants is in the business of improving the quality, access and cost-effectiveness of healthcare for our patients. We do this by developing, implementing, and evaluating plan of care that support the physician’s treatment plan. Nurses provide education and coordinate services for ill or injured individuals and their families. Our goals are to improve the level of wellness and quality of life for our patients and their families, and assist clients in meeting employees’ healthcare needs cost effectivey.

Case Management Consultants
is an innovative company that has been providing services since 1987. Our clients include employers who self-fund their employee’s healthcare. Our company focuses on providing case management services for complex and chronic disease cases.

Services Offered:

• Screening of all referred cases to identify patients at risk.
• Development of individual plans of care that help monitor, educate and coordinate progress of medical treatment as patients move through the continuum of care.
• Promote efficient use of employee benefits by implementing realistic cost containment strategies.
• Utilization management services to pre authorize and certify inpatient and outpatient services.
• Use of a predictive modeling software program to proactively identify high dollar and high-risk cases.
• Educate companies regarding progressive healthcare advances in order to assist them in making decisions regarding healthcare benefit plan design.
